Please provide us with the following information for the patient you wish to contact.
Every effort will be made to deliver this message, however delivery is not guaranteed. Deliveries will be made Monday through Friday (with the exceptions of holidays). Messages containing inappropriate content will not be delivered.
In the event that a message is undeliverable or received after patient's discharge, the message will be shredded or discarded. No record is kept of messages, delivered or undelivered.
